    • utility bill (not phone)
    • rental agreement
    • escrow papers
  3. Copy of Middle School transcript
  4. Copy of Standardized Test scores (SAT-9)
  5. Withdrawal grades from current school

 Arrival and Departure:

Students should not arrive on campus before 7:40 A.M. and must clear campus by 3:00 P.M., unless involved in extracurricular activities. From the time a student leaves home and returns after school, he/she is under the jurisdiction of the school and will be disciplined for any misbehavior to or from school. Disciplinary measures may include counseling, conferences, detentions and/or suspension from school, or other appropriate measures.

Closed Campus:

Yorba Middle School Academy of the arts  has a closed campus. Students are not allowed to leave campus at any time during school hours unless they have permission form the office. Visitors are not allowed on the Yorba Middle School campus.

Telephone:

A pay telephone is provided for student use. Students may use the telephone before and after school. The phone is not to be used during the regular school day. Students should not ask teachers to allow them to use the telephone. If the student has an emergency, he/she should notify the main office for assistance.

Emergency Cards:

Emergency cards are to be completed each year by parents and filed in the office. It is essential that these cards be accurate and up-to-date, in case of an accident or illness. Change of address, phone number, or any other information should be reported immediately to the Attendance/Health Offices. Persons listed on the emergency card should be readily able and willing to assume responsibility for the student in the parent's absence.

Emergency Drills:

Emergency evacuation drills will be conducted periodically. All students should become familiar with the procedure to be followed in each of their classes. Students are asked to report to their current teacher upon reaching the volleyball courts, sit down for roll call, and remain with their class during the entire time of the drill. No talking, running, or pushing is to occur during the drill.

Buses:

Students riding buses are required to maintain discipline and any person whose conduct is unacceptable will be reported. Providing buses is not required by law, but is a privilege to students who cooperate. Students who are discipline or safety problems may lose bus privileges. Bus referrals will result in disciplinary action and/or denial of transportation. Bus passes are purchased and issued at the OUSD Transportation Office, 726 W. Collins, Orange. (714.538.8295)

Lost and Found

Found articles should be brought to the Lost and Found, which is located in the Counseling Office. Students should have identification marks on all of their personal belongings and should repeatedly check if anything has been lost. Unclaimed articles will be donated to a local charity at the end of each trimester.

Insurance

All students participating in athletics are required to have health and accident insurance. It is highly recommended that every student in school have such a policy. Insurance applications may be obtained at the Health Office or from your P.E. teacher.  Off-Campus Policy - Students are not permitted to leave campus for any reason during school hours unless permission has been obtained from the Attendance Office. To obtain an off-campus pass, a note must be presented to the Attendance Office in the morning before school starts, with the specific time of dismissal written on it by a parent or guardian. Students must sign out at the Attendance Window at the time they leave campus, and if returning on the same day, they must sign in at the Attendance Window. Any student leaving campus without prior permission is automatically considered truant.

If You Move/Withdraw From School

The student must report to the Counseling Office with a note stating where he/she is going. A clearance card must be signed by all of the student's teachers and the librarian and returned to the office for the student to leave in good standing. A transfer card will be issued to be presented to the next school. The office must be informed at least one day prior to a withdrawal.

Grading Procedures

In each subject students are graded on academic achievement and given a citizenship rating. Citizenship ratings do not reflect academic achievement, but rather are an indication of a student's willingness to cooperate with classroom procedures and behavior standards. Report cards will be mailed home at the close of each grading period.

Progress Reports

As a caution to the student and an alert to parents, unsatisfactory work notices are sent home whenever it becomes evident that a student's classroom grades may be dropping, or a failure notice will be sent if the student is in danger of failing. These reports do not become a part of the student's permanent records; however, they do serve as warning that there is an immediate need for scholastic improvement. If parents need more information regarding the problem, they should immediately contact the classroom teacher and the counseling office.
